In June 2013, a CS137 industrial source was discovered (specifics ) at an unnamed person's home in Freetown, Sierra Leone. The Nuclear Safety and Radiation Protection Authority transported the source to the Authority's headquarters. The Authority then notified the IAEA’s Incident and Emergency Centre (IEC), who activated the Source Management Unit at NEFW’s Waste Technology Section. Within four days, an IAEA expert arrived in Freetown, who properly conditioned the source, performed a dose assessment, and in conclusion, placed it in long-term storage in Sierra Leone.
