One man has lost his leg and another has been taken into custody following an assault which shut a busy highway in Perth's western suburbs.

Police were called to West Coast Highway near the Boulevard in City Beach at about 11:20am on Monday, following reports of a disturbance.

It's believed two men were involved in a verbal altercation at a nearby construction site.

Police said a 29-year-old man ran away from the area but was followed by a 25-year-old man in a utility vehicle.

Superintendent Manus Walsh said there was still much about the incident that was unknown.

"It is possible that the victim was struck by the vehicle and a secondary assault has then occurred by the offender with the use of a hammer," he said.

Emergency services found the 29-year-old with serious injuries to his head and leg.

He was taken to Royal Perth Hospital, where he remains in a critical condition and is expected to undergo surgery.

"The victim has a severed leg, the lower part of his leg," Superintendent Walsh said.

"He's also received some injuries which we believe have been caused by the use of a hammer."

Two members of the public, including an off-duty doctor, stopped on the highway and applied a tourniquet to the man's leg.

"Had the victim not been rendered medical assistance by witnesses at the scene, there is a likelihood that he would have lost his life," Superintendent Walsh said.

Police located the 25-year-old driver nearby and arrested him shortly after arriving at the scene.

"Within 14 minutes of the call being reported to police, we had an offender in custody," Superintendent Walsh said.

"The two males were known to each other … we're still investigating what the actual relationship is."

The hammer allegedly used in the assault has not been located.

Aerial vision from the scene showed a large pool of blood and other debris in the southbound lanes of West Coast Highway.

Further down the road, a silver utility had crashed in an embankment on the side of the street.

Police were seen scouring through bushland and taping off access paths to the beach.

The busy West Coast Highway was closed in both directions between Hale Road and the Boulevard.

Police asked anyone with information to contact Crime Stoppers.
