During maintenance works at the Borzesti TPP non destructive examination had to be done for some pipes. The examination failed due causes unknown at the time. Further investigations revealed that the pipe NDE failed due to undiscovered initial failure of the Iridium 192 source supporting stem break, followed subsequently by NDE personnel failure to observe the specific procedures for the source manipulation. The source had an activity of 622 Gbq and 6 workers got doses of max. 75 mSv/person. A press release was issued on July 6, 1995, immediately after the irradation was confirmed. Corrective actions are being taken to avoid further stem breaks and failure of the personnel to observe the NDE procedures.
