The Pennsylvania Department of Environmental Protection Southwest Regional Office was notified on November 22, 2013 that a box containing PD103 seeds used for brachytherapy were lost after being disposed of inadvertently, but later recovered. On November 20, 2013, the box containing the seeds was delivered to the Nuclear Medicine Department of the University of Pennsylvania Medical Center in Greenville, PA, where two days later, prior to an implant procedure, a medical physicist noticed that the seeds were missing. A total of 90 PD103 seeds were in the box when it was disposed of. On the same day the seeds were noticed to have gone missing, the medical physicist retrieved the box with all the seeds.
