The licensee, Air Force Radioactive Recycling and Disposal (AFRRAD) notified the manufacturer, AMDS Bioenvironmental Engineering (BE) that they had received the Ionscan, but it did not contain the necessary radioactive source. BE lent its assistance to search for the missing source, but could not find it and instead sent the Unit RSO a picture of the component in question to aid in the investigation. They also requested documentation of the last time the manufacturer received the source, speculating that it could have been removed at that time, but the documentation could not be found either. After extensive efforts, the missing source could not be located.
