The licensee reported to the state the loss of an ionizing static dissipater containing a 10 mCi PO210 source. The licensee properly returned all other devices for destruction from the facility. A subsequent report on May 31, 2013 indicates that the Massachusetts Radiation Control Program conducted a site visit on May 29, 2013, during which the licensee thoroughly searched the facilities and determined the device could have been inadvertently dispositioned during recent building renovations and departmental reorganizations and relocations. The device was last used in 2012. The licensee is also seeking to terminate their general licensing registration because the devices are being phased out for non-radioactive material equipment. All other devices are accounted for and have been dispositioned.
