The licensee (name not provided) notified the Texas Department of State Health Services that a package containing radioactive medical seeds had been lost. The licensee received the package, which had 41.2mCi of I125 seeds, on May 26, 2015 and placed in a designated area in the nuclear medicine department, according to procedure. The staff of this department had been allowed to leave that day, and when they returned the next day they could not find the package. No one else had entered the department according to the licensee's knowledge. The housekeeping employee did not report removing any packages. A search was conducted of the entire facility, but the seeds were not found. The waste container had been taken to the landfill that morning, but the radiation monitoring of the landfill did not detect radioactive material in the waste from the hospital, and had proceeded to dump and cover it. The investigation remains ongoing.
