Radiation Protection personnel performed a required semi-annual source leak check and inventory on April 16, 2013 and could not locate the source label tag or cable for RMC-0169 on its radiation monitor. Additionally, the radiation monitor's pre-amplifier box had also been removed. Despite a search of the monitor and elsewhere, the source could not be located. During the previous inspection, which was on October 4, 2012, the monitor contained the RMC-0169 source. The RMC-0169 source is 200 milligrams of depleted uranium.
