Qualspect Manager employees left a source and camera unattended while conducting a radiography operation. The camera (serial number 4850) containing a 104 Curie IR192 source was left dangling two feet from the floor. An employee from the Valero Benicia Refinery found the camera and returned it to Qualspect employees. The California Radiologic Health Branch (RHB) is conducting an investigation.
