CBRN event in Oklahoma, Broken Arrow area, United States of America on Wed 8th October 2014

On September 30, 2014, the licensee shipped two SPEC-150 radiography cameras each containing less than 57 curies of IR192 to their Broken Arrow, Oklahoma office from a job site in Morgan Hill, California. While one of the cameras was successfully shipped to its destination, the other did not make it, and by October 1, the camera was last known to have been in Memphis, Tennessee. The licensee notified the common carrier, who carried out an investigation of specified dumpsters to determine whether the camera had been accidentally disposed of. When the investigation yielded no results, the common carrier “enlisted more personnel” to advance the search. On October 11, 2014, it was reported that the camera had been found “at the common carrier's facility in San Jose, California where it had been originally dropped off for shipment.” As of this date, the radiography has been returned to the licensee, who affirmed that “the radioactive source is in a safe condition.”
