On September 3, 2014 a CO57 flood source was reported to have been missing for two weeks. Believing the source was accidentally shipped back to the vendor, the licensee contacted the vendor (Eckert & Ziegler Isotope Products, Inc) and determined that the source had in fact “been accidently mailed back to their supplier along with another source.” An investigator for the state of Florida will be sent to evaluate the incident and “provide recommendations,” however the case is now considered closed.
