On September 2nd, a 11.6 Ci IR192 source was delivered to the wrong hospital in Washington DC. The source was meant for Radiation Oncology Center at Sibley Memorial Hospital, but was instead delivered by the carrier to Howard University Hopsital Cancer Center. Sibley Memorial notified the NRC and the carrier, who transported the source from Howard University Hospital to Sibley Memorial the next day. The source is a Elekta Model 105.002 source and its serial number is D36F5755.
