On September 25, 2014, the loss of six depleted Uranium shields was reported by the licensee Mallinckrodt. The shields, which are “used as shielding for TC99m generators,” are thought to have been lost during shipment between 5/2013 and 9/2013. Of the six shields, it is thought that five were lost during their shipment from Mallinckrodt to Triad Isotopes in Hicksville, NY. The Shipping company “Associated Couriers” was responsible for this shipment. The final shield was being shipped from Mallinckrodt to Cardinal Health (Plainville, NY) by “Medical Delivery Services” when it disappeared. According to the report, each shield contains 8.1 mCi of Uranium and “The dose rate on contact would be roughly 3.1 mrem/hr, and the dose at 1 foot would be roughly 1.2 mrem/hr.“
