On September 22, the RSO from WRA (Whitman, Requardt, & Associates) informed the Virginia Department of Health that a gauge containing CS137 and AM241 was lost in shipment. The WRA shipped the gauge to Northeast Technical Services (NETS) for calibration on August 12, 2015. NETS shipped the gauge back after calibration but the WRA did not receive the gauge. After investigations and searches by both sides, later on September 24, 2015, the gauge was found to be delivered to another Virginina licensee by mistake because that licensee also sent a gauge to NETS. The gauge was returned to NETS for inspection and will be sent back to the WRA. The common carrier is investigating how the gauge was delivered to the wrong address.
