On October 27, 2014, a source owned by the Department of Energy (DOE) containing CS137 was lost down a strategic oil reserve cavern while the licensee was taking measurements of the reserve cavern for DOE. Following the source's loss, the licensee employed "all practical applicable P/A procedures pertaining to a cavern." After many attempts to retrieve the source from the bottom of the cavern., the licensee was granted permission to abandon the irretrievable source from the Louisiana Department of Environmental Quality. The source was “declared irretrievable” by the licensee on November 11, 2014.
