CBRN event in Oklahoma, Mustang area, United States of America on Wed 5th November 2014

On November 6, 2014, Hi-Tech Testing Service of Oklahoma reported to the Department of Environmental Quality (DEQ) the temporary loss of an industrial radiography camera holding an iridium source. The Hi-Tech RSO reported that during an overnight radiography session at the Perkins compressor station, the radiography camera was “unintentionally” left at the site by radiographers. The next morning, Hi-Tech’s client discovered the camera at the site, and contacted Hi-Tech. The camera was subsequently recovered by Hi-Tech, and now remains in its custody. According to the report, there is no evidence to suggest that “the camera was tampered with or that there was an overexposure involved.” An investigation into the incident will be launched by DEQ.
