On November 20, 2015, the Radiation Safety Officer found that an I125 seed was lost at Brooke Army Medical Center on November 17, 2015. The seed witnin a specimen was sent to Pathology for sectioning. The Pathology Resident did not pay attention to verify the removal of the seed. Health Physics personnel conducted investigations but could not locate the seed. Corrective actions have been taken to prevent future recurrence.
