On November 20 2014, during an internal check of the SCK.CEN Belgian Nuclear Research Center of BR2 (Belgian Reactor 2), an IR192 source was discovered missing. The SCK.CEN inferred that the missing source was mistakenly sent to a client who was scheduled to receive 2 sources of the same type in the shipping container. After contacting the receiver, it was confirmed that they had received 3 instead of 2 sources in the shipment. The elevated radioactivity reading in the shipment has caused no adverse effects.
