On November 14, 2014, Weatherford International notified the Texas Department of State Health Services that a device containing AM241 had been discovered missing from the licensee’s R&D facility during an inventory conducted November 10, 2014. The source, last confirmed to be in its storage location in December 2013, is thought to have been “swept out with the trash.”
