On November 13, 2014 the Texas Department of State Health Services was notified by the licensee about an industrial radiography camera that went temporarily out of regulatory control when it fell off the licensee’s truck. On November 12, 2014, local law enforcement informed the licensee that an industrial radiography camera containing a I192 source “had been found on the side of a road approximately 3 miles from the licensee's facility.” According to the licensee, the camera was not secured inside the truck prior to departure from the facility, and fell from the truck’s tailgate during its transport to a temporary jobsite. The camera has since been recovered by the licensee without suffering any damage.
