On March 01 2011, external contamination was identified on the handle of a package labeled Radioactive White I (20 mCi of Tc-99m) that was sent from Triad Isotopes Radio Pharmacy in Pennsylvania. The contents and the interior of the package were not contaminated. The cause of the event is believed to be cross-contamination from the radio pharmacy.
