On June 26, 2014, the University of Wisconsin-Madison received a shipment from Tallahassee Memorial Cancer Center Tallahassee FL containing a well chamber, as well as three source holders and an electrometer. On July 3, 2014 an I-125 source was discovered inside the case carrying the well chamber. The source went undetected by the monitoring system but was identified visually.
