On June 25, 2015, Source Production & Equipment Company in St. Rose, Louisiana shipped three brachytherapy sealed sources contained in vials to Worcester Polytechnic Institute in Massachusetts through a common carrier. The package arrived damaged and the container did not have the sources. The package had been resealed with shipping tape. The State of Louisiana has closed their event, while the State of Massachusetts continues to investigate. Massachusetts Department of Public Health conducted searches at receiving licesse facility and carrier facilities but did not find the sources. The search is still ongoing.
