On June 20, 2014 the loss of a Humboldt moisture density gauge containing a 1.48 GBq (40 mCi) Am-Be source and a 0.3 GBq (8 mCi) Cs-137 source was reported to the Texas Department of Health by the licensee [Nuclear Fuel Services INC.] The loss occurred when a technician working a job site on June 19, 2014 did not successfully secure the gauge on the back of his truck before returning home and also left the tailgate down. As a result, the gauge fell from the truck during the technician’s drive home. On June 28, the licensee reported that the gauge had been found, and after passing a leak check it was determined to be undamaged. Two violations were cited in this incident.
