On June 11, 2013, the state of North Carolina conducted an inspection of Metal Industries, which went out of business on June 1, 2012. During the inspection, it was discovered that a missing item from the June 13, 1995 inventory was not ever disposed of. Follow-up of the inspection with the distributing company (CMI) and the Radiation Safety Officer revealed that the device had not been disposed. However, the state of North Carolina considers the incident to be a closed case.
