On July 31, 2015, Fleet Readiness Center East (FRC East) Radiation Safety Officer (RSO) received a notification of a missing H-53E ice detector probe. The probe contains 50 µCi of SR90, sealed in a metal casing. The probe was last seen in the Aircraft Line Fiberglass Repair Shop on November 4, 2014. It is assumed that the probe has been disgarded as trash and thrown away. Corrective actions have been taken, including setting up a mandatory task line and a training course.
