On July 30, 2015, the NEA Baptist Memorial Hospital, in Jonesboro reported a misrouting of an IR192 source. On July 29, 2015, the source was deliverred by a common carrier to a wrong address with the similar name as the hospital and remained there for 23.5 hours. On the morning of July 30, the source was delivered to the hospital. On September 3, 2015, the hospital submitted a report outlining corrective actions to deal with similar situations. This case was considered closed.
