On July 3, 2014, the Texas Department of Health was notified by the licensee that a moisture density gauge containing an 8 milliCurie CS137 source went missing when a technician failed to properly secure the gauge for transport during a delivery between the licensee’s Austin facility to its facility in Buda, Texas. Upon his arrival in Buda, the technician found “the tailgate on the pickup was down and the gauge was missing.” Following a retracing of the route, the gauge remains missing, however a water jug that was also in the back of the truck was recovered. According to the licensee, the device was not locked, and “there were no locks on the transport case lid.”
