On July 22, 2015, TLS SYSTEMS, LLC reported the missing of a tritium source probably on July 12, 2015. The source was removed from a TLS Model 42000 drogue light and put in a solvent with silicone. The licensee found the source was missing when performing an inventory and thought that the source was rejected as regular trash. The case is currently under investigation by Arizona Radiation Regulatory Agency.
