On July 12, 2013, a breast localization procedure using a seed source took place at the Rockwell Park Cancer Institute. Following the procedure, no one returned the seed to the Nuclear Medicine Department, which led to a search for the missing seed. Unable to locate the seed, the licensee declared the radioactive material officially lost on August 29, 2013.
