CBRN event in Pennsylvania, Mt. Lebanon area, United States of America on Mon 30th June 2014

On July 1, 2014, the Pennsylvania Department of Environmental Protection and Bureau of Radiation Protection was notified by the licensee that a nuclear density gauge last used on Friday June 27th had been lost or stolen. According to the licensee, an employee had used the gauge, placed it back in its case, and then returned it to the Job Site storage area. On Monday June 30th, the employee reported that “the case was now empty.” Following an inspection, the licensee determined that “the case and lock were both intact inside the locked storage area” with no signs of forced entry. Although questioned a number of times, the employee insists the gauge and its case were placed back in the storage area on June 27th. Subsequent searches have failed to identify the device’s location. The incident was reported to local authorities and the licensee has offered a reward for its return. The Department plans to lead a “reactive inspection” of the site.
