On January 21, 2014, the Quaker Oats Co., in Dallas, Texas reported the loss of a process gauge after conducting a routine inventory. Quaker Oats stated that the gauge was removed from the production line and placed into storage several years ago, following proper safety procedures. The gauge's source activity was 100 mCi AM241.
