On January 20, 2012, the U.S. Air Force conducted leak tests on all bioenvironmental engineering air particulate detectors at Fort Meade, Maryland. After the test, the Air Force placed all of the air particulate detectors in storage. On October 17, 2013, the Air Force conducted a program review and determined that a Smith's Detection APD-2000 was missing. The Air Force commenced a records search and discovered that a recent overhaul to the records system prevented location of the missing item. The device's source activity was 10 mCi NI63. On January 30, 2014, the Air Force reported successful location of the device.
