On February 25, 2013, Dameron Hospital in Stockton, CA, received four Mick applicator kits containing 15 I125 seeds in each applicator, for a total of 60 seeds. Each seed contained .45 mCi. The next day, 49 seeds were implanted during the medical procedure, with one seed jamming in the applicator. Upon completion of the procedure, the radiologist noted that only 50 seeds were in the applicator, indicating 10 were missing. The missing seeds were not found after searching. The licensee is investigating the incident, and has so far accounted for all 60 seeds up to the transfer from supplier to licensee to delivery to the hospital.
