Norton Hospital in Louisville, KY reported the loss of an I125 localization seed to the Kentucky Department of Radiation Control. The loss was discovered after an inventory revealed the number of seeds in storage did not match the number of seeds recorded. It is believed that the seed was most likely disposed of in the trash instead of following proper protocol. Corrective actions have been taken.
