Nanticoke Memorial Hospital in Seaford, DE shipped two packages with radioactive materials to Sanders Medical Products, Inc. in Knoxville, TN for disposal. One package was received and the other was declared lost on February 13, 2015. The lost package contained two GE68 source rods. On June 29, 2015, the package was returned by the carrier, unopened and undamaged, to the hospital that shipped it. There was no explanation regarding where the package was or how it was returned. The hospital will use a different carrier to resend the sources to Sanders Medical Products soon.
