A dose of RA-223 was reported lost from a hospital in Spain. A healthcare worker was preparing to administer it as part of a medical proceedure and discovered that the material was missing. Following a search of the premises and interviews with the staff it was determined that the material was probably accidentally disposed of. At the time of the report the hospital is still investigating the incident.
