It was reported on June 18, 2013 that a delivery of radioactive isotopes failed to be loaded onto a flight by Australian Air Express. Telemed director of nuclear medicine Peter Tually spoke with a representative from Australian Air Express the day before the scheduled shipment to ensure that the isotopes were loaded. However, it appears that staff members loading the flight mistook a separate shipment of non-medical isotopes for the Telemed package. A similar incident occurred in February.
