On April 21, 2014, radioactive source IR192 was lost en route from Koirala Memorial Cancer Hospital in Nepal to the capital, Kathmandu. 15 kilograms of the iridium source, used in the treatment of gamma radiation by brachytherapy, was on its way to Belgium in a secured white container with radiation logo via Flight Express when it was lost. It was assumed to have gone missing when en route by bus from Narayangadh to Kathmandu until May 01 2014 when it was found by the owner of a tea shop in Kalanki, Kathmandu. The radioactive source was later returned to the Air Light Express.
