In 2006, the licensee, Fabri-Form company, acquired NRD LLC manufactured ionizers with Polonium sources. The licensee became aware that the devices fell under the United States Nuclear Regulatory Commission rules and regulations on March 31, 2015, which led to a series of communications between the licensee and the point of contact at the NRC. After having gone through a series of attempts at determining the location of the 6 missing devices, as of June 2, 2015, no sources have been recovered and the investigation is still ongoing.
