Genpac, LLC in Middleburg, NY reported the loss of two nuclear thickness gauges. In February 2015 the Genpac, LLC requested a license termination, declaring that they did not have gauges with radioactive materials. The Radiation Safety Officer left the company in 2010 without the licensee notifying the New York State Department of Health, and the licensee stopped maintaining inventories and leak testing at that time. The licensee was unable to provide documentation that accounted for the two gauges going to an authorized recipient. The gauges were not located during a consequent inspection by the New York State Department staff on March 10, 2015. The licensee notified the New York State Department that after interviewing staff and searching the facility, the gauges have still not been found.
