During the modernization of the coal fired Power Plant "Belchatow," the loss of two radioactive sources containing the isotope CO60 was discovered and reported. The sources were placed in their shielded containers. The likely reason for the loss was the lack of proper supervision over the work of the external company carrying out the modernization.
