During an annual inventory audit by the Illinois Emergency Management Agency (the Agency), the licensee discovered that its hand held thin metal alloy analyzer, a “triple head” unit containing three sources, was missing. The unit, which had been “replaced by a newer x-ray producing unit a few years ago,” was saved for backup purposes. Although an investigation has been launched, the unit is thought to have been thrown away and “junked” with other equipment. Currently, the unit does not pose a threat to public health and safety as “the radioactive iron and cadmium sources are decayed to less than milliCurie quantities and have a negligible radiation profile associated with them.” In the absence of additional developments, the Agency “is considering this matter closed.”
