During a source exchange on a Varian HDR, the user noticed high levels of radiation as a result of an unintentional release of material which contaminated the device. The device was placed in a vault and the area cordoned off. The manufacturer was informed and dispatched a technician who isolated the source of the leak, cleaned the area, and disposed of the contaminated material.
