At Duke Medical Center, a recent facility inspection revealed that a lost I125 seed (National NMED Item #140177) was found months later still in a patient's breast tissue. The seed was thought to be removed with the targeted tissue during surgery. Duke Medical Center "maintains that there was only 12.5 rads received to the 250g of breast tissue and not above the 50 rem for medical event reporting". Although the loss and recovery of the source occurred in 2014, no updates or notifications were officially given to the Nuclear Regulatory Commission until May 01, 2015.
