An Iridium-192 source was lost during shipment. The source contained 3.792 Curries of material and failed to arrive at its destination. When this happened, the licensee checked the package tracking information and noted that the tracking information placed the package at the carriers facility. The licensee contacted the delivery service who conducted a thorough search upon learning they had a box of radioactive material lost in their building. Following an intense, multi-day search, the item was located and delivered to its intended receipient.
