An industrial radiography source was reported as missing when it failed to arrive in a scheduled shipment to Oregon Washington Laboratories LLC. The source was found at the commercial carrier's Memphis, TN shipping hub, where it had been placed with other 'lost dangerous goods' because the carrier lacked a copy of the shipping papers to send it to its destination. The licensee delivered a copy of the papers, and the source was shipped as planned.
