An I125 seed (Best Medical, Model Number 2301), which was surgically removed from a patient along with a specimen, was reported missing on November 23rd, 2015 in Pittsburgh, pennsylvania. The specimen was placed in a plastic bag once excised from the patient and sent to the main hospital pathology department. The licensee who discovered the missing seed believes that it was inadvertently thrown away. A search was conducted in the surgical center but the seed could not be located. Further inspections are planned.
