A shipment of IR192 High Dose Rate (HDR) brachytheraphy source was successfully delivered to the Rhode Island Hospital. However, the physics staff members did not respond to picking up the package and bringing it to a secure location. As a result, a staff member moved the package to an unsecure hold room, where it stayed for 11 days total. Hospital staff and Radiation Safety personnel established official new actions to prevent this type of event's recurrence.
