A radioactive I125 seed was lost in the Theragenics strand lab upon preparation for shipment. Searches of the lab and of personnel who had entered the lab were unsuccessful in locating the seed. The strand lab underwent a semi-annual cleaning from December 19th, 2014 to December 20th, 2014 in order to look for the missing seed and another missing seed from September 2014, but no findings have yet been reported.
